RN Case Manager - Diabetes Education

Part-time 24 hours a week

The RN Care Manager is competent regarding both knowledge and skills to create individualized self-management plans, coordinate care, interpret clinical data, conduct focused and or complete educational assessments for patient population (diabetes). The RN will collaborate with all members of the healthcare team to assure transition to the appropriate level of care. This RN will also have the opportunity to educate and coordinate care for patients with diabetes and associated co-morbidities across the health system in order to promote self management skills and reduce hospitalizations.

Minimum Qualifications:

• A Bachelor's Degree in Nursing and licensed to practice nursing in the State of Connecticut.
• A minimum of 7 years of experience in acute care, home care or long term care plus experience in care coordination/management.
• Willing to meet requirements to apply for NCDBE-CDCES certification within 12 months of hire.

Preferred Qualifications:

• Masters Preferred

• Certification in a nursing specialty related to Diabetes, National Certification Board for Diabetes Education as a Certified Diabetes Care and Education Specialist or (NCDBE-CDCSE) is preferred

• Care Management or Care Coordination experience is desired.
